Rejuvenate Integrative Functional Health Clinic Fee Schedule
Initial consultation and care planning- $250 (during this visit we will go over your intake forms, past history, medical history, and symptoms to develop a plan that may include laboratory testing, supplements, and/or medications)
Follow up 60 minutes- $100
Sick visit- $50
Bio-Identical Hormone Replacement Therapy with a functional approach- Initial consultation is $250; Ongoing price varies depending on your custom plan of care.

Weight Loss
We offer a comprehensive weight loss program. This may include medications, coaching sessions, meal planning, exercise recommendations, and other modalities depending on your specific needs,
